First Visit

Upon arrival at Stephen Chimes Dentistry in Rose Bay you will be greeted by our friendly staff who will request you hand in your New Patient Form (if not already electronically completed – please click here for the new patient form). We use all patient information in accordance with State and Federal law; go here to see our full privacy policy. 

Following on from this you will have a 90 minute appointment; the Dentist will perform a Comprehensive Dental Assessment, followed by an Oral Hygiene Treatment with our Hygienist. This will include:

  • 2 dental assessment x-rays (if required)
  • Treatment planning
  • Digital imaging
  • Professional clean and polish
  • Periodontal charting
  • Oral hygiene instructions
  • Fluoride application

Your medical and dental history

As a new patient to Stephen Chimes Dentistry we ask you to fill out a medical and dental health form while at the practice. However, for your convenience we have made it available online so you can download and print it in advance and email/fax it in or bring them into our practice for your first appointment.

All the information is held in strict confidence and will be used to assist in your dental assessment and treatment planning.
